The name of the surging surgical technique which is almost replacing the need for total knee replacement is ‘High Fibular Osteotomy’ or “Proximal Fibular Osteotomy”. It is a simple alternative which produces dramatic, surprising & unexpected results” & here I take liberty to quote Chennai based noted Orthopaedic Surgeon, Dr Lakshman Prakash.
I am writing this for the benefit of the thousands of friends on FB, who themselves, or their friends, or relations are contemplating a Knee Replacement, or, are fearing visiting the orthopaedic specialist to avoid having to contemplate it.
We as knee specialists, have been trained to tell the patient, to come for a knee replacement as a last resort, when all else has failed, & pain & immobility has made life miserable.
Long long before such a situation arises, when the knee is beginning to lose alignment and the patient is developing a lurch in the gait, a PFO will work by shifting the centre of gravity away from the inner part of the knee to a more neutral area towards the centre of the top of the tibial plateau.
A small cut in each leg & just half an hour – and lo & behold ! Your excruciating knee pain disappears to your great sigh of relief & you walk back home in the evening same day quite normally after the minimal intervention.
“Proximal Fibular Osteotomy (PFO) is a rather simple surgical procedure” says Prakash, “involving removing a centimetre of fibula some eight centimetres below the fibular head. And somehow this procedure produces dramatic relief from pain due to medial compartment OA in selected cases”.
In his new book on “PFO for Osteotomy for Medial Compartment Osteoarthritis of the Knee”, Prakash elaborates on how “Isolated medial compartment knee arthritis, with varus deformity has remained an unsolved enigma & an intriguing problem to the orthopaedic surgeons for ages”.
He states, how “These have been treated in the past by conservation, high tibial osteotomy (HTO), or Unicompartmental knee arthroplasty (UKA)”. Deploringly, he mentions “Conservation is not always successful, & both HTO & UKA are major procedures, & complications if any can be disastrous at worst.”
In fact, despite there being such a simple, cheap & efficacious technique as PFO, knee replacement is being thrust upon patients pf knee OA. Only one percent of a thousand needs knee replaement, the rest can be effectively taken care of by PFO. But 50% of them are advised knee replacement, out of which 10% fall for it. The total knee replacement is only for patients in the 4th or end stage of OA.
In Japan, China & other countries, PFO or HFO as it is called, is fast becoming the first line of treatment of OA, not knee replacement. Even patients aged 70 are benefitiing from this simple treatment. Clinical evidences of thousands of people vouch for hundred percent success.
